Registration Form & Code of Conduct

Players and parents/guardians should read and understand the Code of Conduct [available here] and then download and complete the Registration Form [available here].  The Form needs to be signed by both the player and parent/guardian.

Please give the completed Form together with payment in a envelope clearly labelled with the player's name to the Team Manager at this weeks training session.  Alternatively payment can be made by electronic transfer at the details below.
